Accessing Your Health Information

At St. Joseph Hospital, we are committed to assisting you with your requests for proof of birth, death, or admission efficiently and transparently. Please note that the hospital does not issue birth or death certificates. After a birth, we provide a Statement of Live Birth, which is a vital record required to apply for a birth certificate through your local registrar’s office or government portal. Similarly, for death certificates, the hospital issues a Medical Certificate of Death, which is sent to the funeral home handling the arrangements. The funeral home then registers the death, and you can obtain the official death certificate from the appropriate government office. If you require proof of hospital admission for personal or insurance purposes, our Records and Information Services Department is available to help, provided you present the necessary identification and authorization. Additionally, if you believe any personal health information in your hospital records is inaccurate or incomplete, you may submit a Request for Correction of Personal Health Record form to our Release of Information Department, allowing up to 30 days for processing. Our dedicated team is here to guide you through these processes and ensure all your documentation needs are met.
